Background
The Southside Coalition of Community Health Centers is a non-profit organization established to sustain, coordinate and improve health care access and delivery to the impoverished and vulnerable community members of South Los Angeles. We are a network of eight Federally Qualified Health Centers (FQHC) that are either located in South Los Angeles or serve a substantial portion of patients who live in South Los Angeles. Our collaboration creates collective strength that builds capacity, coordination and networking that focuses on the patient and supports quality, culturally and linguistically proficient delivery of care.
Overview
The Community Health Worker will conduct outreach, education and engagement activities for the patients receiving care at our member clinics and into their surrounding communities in South Los Angeles. Specifically, this position will work to assist individuals in understanding California and federal health insurance programs they may be eligible; will conduct screening, eligibility, and enrollment assistance into health coverage programs when relevant; provide education on the appropriate use of medical services through the use of a medical home; and will make referrals for community residents currently without a usual source of care to a medical home. Additionally, this position will assist with outreach to patients who are behind on annual health screenings or need assistance with scheduling appointments with their primary care provider. This is an hourly non-exempt onsite position with no remote work. Work hours: Monday – Friday 8:00 am – 4:30 pm. This position is a temporary two-year grant funded position.
Responsibilities
Community Health Worker duties include:
- Conduct outreach/education activities at community health centers, or within other community settings to provide education on the appropriate use of a medical home and to education community members on various options available for health care coverage.
- Facilitate referrals to appropriate health care services to community members that are not connected to a medical home.
- Conducts eligibility screening and application assistance to enroll eligible patients into various health coverage programs.
- Answer questions about the reauthorization process/renewals on an annual basis to retain coverage for patients.
- Attend meetings as required for updates.
- Collects data on outreach activities and submits monthly reports.
- Participate in local health/community fairs to promote program services.
- Assist with outreach to patients of our member health centers.
- Distribute brochures, flyers and educational materials to communities.
- Other duties as assigned by Supervisor.
Minimum Qualifications and Experience Requirements
- High school/GED required. Associate’s or Bachelor’s degree a plus.
- A demonstrated commitment to working with families living in disadvantaged communities, of diverse racial and ethnic backgrounds, is essential along with a familiarity of the health disparities and challenges.
- Good verbal and written communication skills and the ability to convey information clearly to others.
- Basic computer and internet skills required for routine data collection, data entry and reporting; experience with Microsoft Word and Excel Spreadsheets.
- Excellent time management skills and ability to complete tasks on time; ability to multi-task.
- Willingness and aptitude to work effectively either in a team based setting or independently.
- Must pass background check.
Pay: $23.00 - $24.00 per hour
