Are you passionate about helping families navigate the aging journey? Hera is seeking a Geriatric Intake Specialist to join our team on a 1099 contract basis (up to 35 hours/week). This role is fully remote and offers the opportunity to make a meaningful impact every single day.
As the first point of contact for families, you will be the “face of Hera” building rapport, guiding them through options, and ensuring a seamless onboarding experience. You’ll play a dual role:
- Build trust with new clients by sharing Hera’s value and helping families see how our high-touch care management can support them.
- Coordinate intake and logistics between patients, their families, Hera’s care team, and external providers making sure every client transitions smoothly into their first appointment.
Responsibilities
- Build trust & provide support: Establish rapport with clients and families, offering supportive care and structured plans that ease stress and build confidence in the process.
- Guide families through intake: Serve as the first point of contact for new patients, helping them complete paperwork, gather documentation, and prepare for their first Hera appointment.
- Navigate care logistics: Coordinate between patients, Hera’s in-house team, and external providers to ensure a seamless handoff.
- Support technology adoption: Help patients and families navigate technology (uploading documents, accessing the platform, scheduling appointments).
- Collaborate with BPO partners: Work closely with our outsourced team to streamline workflows and scale intake capacity.
Skills
- 4+ Years in social work, care coordination, patient navigation, client services, or community health. (MSW, BSW certification, or equivalent experience preferred but not required).
- Excellent relationship management & communication skills → you balance compassion with boundary-setting, and families trust you as credible and competent.
- Strong operational discipline → you use checklists, trackers, and workflows to stay on top of details.
- Comfortable with technology and EHRs → quick to learn new platforms (task management, scheduling, CRM).
Job Type: Contract
Pay: $25.00 - $40.00 per hour
