Job Summary
Jessie Trice Community Health System in partnership with the Children's Trust is seeking a dedicated and compassionate Community Health Worker/Driver to support our schools and community initiatives. This vital role involves providing health education, assisting individuals with accessing healthcare services by helping with appointment scheduling, form completion, and follow-up on referrals. Additionally, CHWs perform administrative tasks including patient registration, collecting insurance information, updating medical records, scheduling specialty appointments, and coordinating with pharmacies regarding prescriptions. They also collect patient medical histories in person or through Telehealth and assist healthcare providers during visits or procedures, contributing to a more efficient and patient-centered care experience.
The ideal candidate will possess strong interpersonal skills, a commitment to public health, and the ability to navigate diverse community settings. As a Community Health Worker/Driver, you will serve as a bridge between healthcare providers, schools and community members, promoting health awareness and improving access to essential services.
Requirements:
High School Diploma
Ability to seek out assistance and information, when necessary, competent infection control skills and knowledge.
Proficient with computer and working knowledge in Electronic Health Record (EHR).
Excellent communication & people skills.
Demonstrated ability to work effectively with diverse populations.
Experience in data collection is helpful in maintaining accurate records.
Motivated self-starter with a compassionate approach to community service and health promotion.
Pay: $20.00 - $21.00 per hour
