We are seeking a Care Coordinator who thrives at the intersection of empathy and execution. You’ll quarterback patient cases managing referrals, benefits, and logistics while also supporting families through some of the most sensitive transitions in care. This role requires equal parts compassion, operational excellence, and relentless follow-through.
The Role
- Partner closely with Hera’s Care Managers to execute personalized care plans for older adults and their families.
- Manage a caseload of patients, coordinating across physicians, home health, and community services.
- Support referrals for specialists, DME (e.g., oxygen, walkers, hospital beds), and medication packaging (blister packs).
- Help families navigate advance care planning, Medicare/Medicaid benefits, and transitions between home, rehab, and facilities.
- Build and maintain structured trackers and workflows to ensure no detail is missed.

- Bring 2+ years of direct experience in care coordination in geriatrics, palliative care, or hospice services.
