Position Available: Full Time Home Health and Hospice Scheduler
Position Summary
The Scheduler is responsible for coordinating and maintaining the daily visit schedule for all clinical staff across Home Health and Hospice. This role ensures patients receive timely, appropriate care while supporting clinicians with accurate routing, communication, and schedule adjustments. The scheduler acts as a central communication hub between patients, families, clinicians, referral sources, and leadership.
Key Responsibilities
Scheduling & Coordination
- Build, assign, and manage daily schedules for RNs, LPNs, PTs, OTs, STs, MSWs, CNAs, and Chaplains.
- Ensure all admissions, routine visits, ROC, recerts, and SOC visits are scheduled appropriately and within required timeframes.
- Adjust schedules throughout the day for call-outs, priority needs, hospital discharges, urgent hospice situations, and changes in patient condition.
- Notify clinicians of all updates in real time.
Communication & Support
- Serve as the primary point of contact for scheduling questions from patients, families, and staff.
- Communicate with referral partners for urgent start-of-care or hospice admissions.
- Maintain professional, calm communication—especially during high-volume or crisis moments.
Operational Compliance
- Schedule visits according to state, federal, and agency requirements.
- Ensure discipline frequency orders are followed and updated when changes occur.
- Track recurring orders and physician-specified visit patterns.
- Prepare daily huddles or reports for leadership and clinical teams.
Documentation & Systems
- Add, update, and maintain visit statuses within WellSky (or agency EMR).
- Track missed visits, cancellations, and no-visit documentation needs.
- Monitor capacity, mileage considerations, and clinician case loads.
- Assist with on-call report preparation and weekend coordination.
Qualifications
- Experience in Home Health or Hospice preferred.
- Strong understanding of disciplines, visit types, and care timelines.
- Excellent communication and multitasking skills.
- Ability to stay organized under pressure.
- Proficiency with EMRs (WellSky experience highly preferred).
