JOB SUMMARY
The Community Health Worker will play a vital role in supporting psychological testing services by coordinating communication between families, schools, healthcare providers, and internal teams. This individual will manage administrative workflows including scheduling, documentation, and prior authorizations, while also providing compassionate follow-up to ensure families successfully connect to recommended services such as therapy, school supports, and ABA services.
This position requires strong organizational skills, attention to detail, and the ability to engage families with empathy and professionalism.
JOB DUTIES
Care Coordination & Family Engagement
- Follow up with families to obtain signed consent forms for psychological testing
- Maintain ongoing communication with families throughout the evaluation process
- Conduct post-feedback follow-ups (approximately 30 days) to ensure linkage to recommended services (e.g., school supports, therapy, ABA)
- Provide guidance to families on next steps after evaluations
Administrative & Documentation Support
- Complete and track prior authorizations, including submission, scanning, and updating patient charts upon approval
- Manage documentation workflows including scanning, faxing, and Health Information Management (HIM) processes
- Monitor Behavioral Health desktop for incoming referrals and coordinate accordingly
- Ensure all documentation is accurate, complete, and submitted in a timely manner
Scheduling & Workflow Coordination
- Schedule appointments including intakes, testing sessions, and feedback appointments
- Coordinate with internal teams and providers to ensure efficient scheduling and minimal delays
- Track outstanding items such as teacher assessments and follow up as needed
Clinical Support Coordination
- Follow up with families and schools when teacher assessments are incomplete or delayed
- Review previous evaluations and IEP documentation (as applicable) to support ABA referrals (e.g., Spectrum of Smiles)
- Collaborate with HRDI and other internal departments on referral coordination
Front Desk & Patient Support (as needed)
- Assist with patient check-in processes (e.g., Thursdays), including verifying and updating insurance information
- Ensure a welcoming and supportive experience for patients and families
QUALIFICATIONS
- Completion of a GED or high school diploma.
- Completion of (CHW II) or enrollment in (CHW I) a CHW certification program.
- Minimum of 2–3 years of experience in customer service, outreach, or other community-facing roles, preferably in healthcare, public health, or social services.
- Experience working with families, children, or behavioral health populations preferred
- Knowledge of local population demographics, assets, and needs.
- Knowledge of local community health disparities.
- Ability to establish patient connection and provide ongoing support in behavior change.
- Demonstrated organizational and time management skills.
- Effective written and oral communication skills to document patient information and progress.
- Computer literate; proficient in Microsoft Word, Excel, and PowerPoint.
PHYSICAL REQUIREMENTS
- Moderate physical activity may include lifting 25–50 lbs.
- May require extended periods of standing or walking.
- Minimal exposure to physical risk.

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Employees who decline TB testing will be required to wear a mask when deemed necessary.
