Responsibilities
- Engage with community members to assess their health needs and provide culturally sensitive education on topics such as nutrition, medication management, addiction counseling, and preventive care.
- Assist individuals with navigating healthcare systems, including Medicare enrollment and understanding medical terminology.
- Conduct outreach activities through public speaking engagements, workshops, and one-on-one coaching to promote healthy behaviors.
- Collect and document data related to health outcomes, service utilization, and community feedback to support program evaluation and improvement efforts.
- Collaborate with healthcare professionals, social workers, and local organizations to coordinate services for individuals with disabilities or complex health conditions.
Experience
- Demonstrated ability to work effectively with diverse populations, including individuals with disabilities or those facing socioeconomic challenges.
- Strong knowledge of medical terminology, addiction counseling principles, and nutrition fundamentals.
- Ability to build rapport quickly, demonstrate cultural competence, and maintain confidentiality at all times.
