Responsibilities

Engage with community members to assess their health needs and provide culturally sensitive education on topics such as nutrition, medication management, addiction counseling, and preventive care.
Assist individuals with navigating healthcare systems, including Medicare enrollment and understanding medical terminology.
Conduct outreach activities through public speaking engagements, workshops, and one-on-one coaching to promote healthy behaviors.
Collect and document data related to health outcomes, service utilization, and community feedback to support program evaluation and improvement efforts.
Collaborate with healthcare professionals, social workers, and local organizations to coordinate services for individuals with disabilities or complex health conditions.

Experience

Demonstrated ability to work effectively with diverse populations, including individuals with disabilities or those facing socioeconomic challenges.
Strong knowledge of medical terminology, addiction counseling principles, and nutrition fundamentals.
Ability to build rapport quickly, demonstrate cultural competence, and maintain confidentiality at all times.
