Job Overview:
We are seeking a dedicated and knowledgeable Health Educator to join our team. This role involves developing, implementing, and evaluating health education programs aimed at improving community health outcomes. The ideal candidate will possess strong communication skills, a background in public health or related fields, and experience working with diverse populations, including individuals with disabilities. Essential Functions and Responsibilities:
- The Health Educator is responsible for seeing patients for Comprehensive Perinatal Services Program (CPSP), Family Planning (Title X/Family PACT), Every Woman Counts (EWC), and The Wise Woman grant.
- Process program applications for Every Woman Counts program and provide DETEC case management services, ensuring compliance with program guidelines and facilitating timely follow-up and care coordination.
- Process applications Under the Family PACT program and administer family planning educational sessions in compliance with Title X protocols, and provide family planning counseling, ensuring clients receive accurate information and timely access to services in accordance with program guidelines.
- Under the WISEWOMAN grant, the Health Educator will deliver culturally competent case management, including client enrollment, screenings, care coordination, health education, and referrals, while ensuring follow-up, tracking outcomes, and maintaining accurate documentation in accordance with program guidelines.
- Responsible for ongoing prenatal/postpartum health education services on a one-on-one basis or in group sessions.
- Establish and update local resources and referral networks that are not available from providers for Perinatal services such as smoking cessation programs, childbirth education classes, hospital tours, parenting support groups, adolescent pregnancy services, dental, and substance abuse programs annually, etc.
- Responsible for making attempts to retain all scheduled patients through following up with patients’ subsequent visits, making calls to regain patients, and/or tracking patients through various reports and finding means of contacting them.
- Attends in-house trainings and/or external trainings in order to improve and extend their knowledge and services provided
- Provides comprehensive education and information through community outreach, health fairs, school relationships, community events, and other approved activities per Title X Requirements
- Communicates any found needs to the WHD department through the use of the Individualized Care Plan Documentation, EHR intake documentation as well as over the phone and/or email· Review and revision of the Health Education materials for CPSP and Family Planning/Title X annually
- Prepares and disseminates posters, audio-visual materials, etc., that will be used in the health education program.· Perform all other duties and tasks assigned by the supervisor or manager
Education:
Bachelor’s degree in public health education
Experience:
One (1) year experience in health education and coordinating health-related projects.
