Worker is passionate about advocating for clients in the community. Effective communication skills for interactions with clients, family members, and health care providers is an aspect of this work. The ability to maintain confidentiality while problem solving potential issues related to the health care system, financial, and social barriers is beneficial to this role.
For a typical assignment, the Community Health Worker will receive referrals from various paths including health care providers, insurance companies, Accountable Care Organization (ACO), managed care plans, and other organizations working to help manage individuals' social health needs. You will manage a caseload of referred clients/patients. You will coach individuals to adopt healthy behaviors, help them access community services and resources, and facilitate communication with health care providers and social service organizations. Through an integrated approach to case management and care coordination, you will provide social support to vulnerable populations. Job posting will remain until filled.
