Job Overview
We are seeking a dedicated Community Health Worker to join our team. The ideal candidate will play a crucial role in promoting community health and well-being by providing support, education, and advocacy to individuals and families. Schedule will be Tuesdays 10am-6pm, Friday 10am-6pm, and Sundays 8:30am-2:30pm in Wayne, MI. *Please note this role is subject to grant funding allocations and annual renewals.
Duties
- Collaborate with healthcare professionals to develop and implement community health programs
- Conduct outreach activities to connect community members with healthcare services
- Provide health education on topics such as preventive care, nutrition, and chronic disease management
- Assist individuals in navigating the healthcare system, including understanding insurance
- Utilize motivational interviewing techniques to encourage healthy behaviors
- Collect and maintain data on community health needs and outcomes
- Advocate for underserved populations to ensure access to quality healthcare services
Requirements
- Background in social work, public health, or related field
- Strong communication skills with the ability to engage in public speaking engagements
- Knowledge of medical terminology and healthcare systems
- Ability to educate individuals on health-related topics effectively
- Familiarity with data collection methods for monitoring health outcomes
- CHW experience required; Core Competency Training preferred
