Health Program Specialist III

The Division of Services for People with Disabilities is seeking a dedicated professional for the Health Program Specialist III role. This position provides ongoing support coordination and case management for individuals receiving DSPD services. The Health Program Specialist III works directly with people and their teams to assess needs, develop person centered plans, and ensure that services are delivered appropriately and consistently. The role includes coordinating providers, monitoring service effectiveness, addressing barriers, and helping individuals maintain stability in their home and community. This position focuses on service oversight, problem solving, crisis coordination, and ensuring that supports remain aligned with each person’s goals, preferences, and safety needs.

Resolves a variety of complex issues and may perform a combination of the following tasks. Develops, interprets, and implements policies. Ensures enforcement and compliance with federal and state rules, regulations, and mandates. Provides quality assurance and monitors program effectiveness. Provides education, communicate and educate contracted community providers and staff, care recipients, and the pubic. Work closely with other state agencies on Medicaid eligibility.

Responsibilities

Provides clients with information concerning rights, options, benefits, services, goals and expectations.
Ensures compliance with applicable federal and/or state laws, regulations, and/or agency rules, standards and guidelines, etc.
Receives, researches and responds to incoming questions or complaints; provides information, explains policy and procedures, and/or facilitates a resolution.
Refers client, partners, and families to other available services to meet needs where appropriate.
Enters data into a computer system and retrieves, corrects, or deletes previously entered data.
Proficient in word processing documents like word, excel, etc…
Reviews and/or inspects work for quality, accuracy, and completeness.
Collaborates with other state and local programs, organizations and agencies, and with various stakeholders to promote program goals and objectives.
Respond to inquires for community based services.
Manage crisis situations.
Support and sustain DSPD goals, plans, and objectives.
Demonstrate leadership
Apply understanding of a person's eligibility for services.
Negotiate the DSPD budget worksheet within the individualized budget.

Qualifications

Is adept at interacting professionally with all levels of management, and dealing with people in a manner which shows sensitivity, tact, and professionalism.
The ability to multitask effectively by switching between tasks, screens, and duties.
The ability to manage time effectively.
Demonstrate strong professional writing and communication skills.
Has experience in facilitating cross-agency collaboration.
Experience in working with individuals with intellectual disabilities.
Preference will be given to people who have worked with agencies such as Medicaid or Department of Workforce Services.
Has experience supporting individuals with disabilities to lead self-determined lives.
Has experience in Person-Centered thinking and processes.
Has knowledge of Home and Community-Based Services (HCBS).
Understands Department waivers and resulting contracts with direct service contractors.
Understands Division services.
Has strong interpersonal skills.
Has a strong understanding of cultural competence.
Has the ability to work well independently..
Is an excellent listener who is also proactive.
Has the ability to work well with a wide range of people with varying interests.

Supplemental Information

Risks found in the typical office setting, which is adequately lighted, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
This position is currently a hybrid of both in-office and remote work days. Please note, a position's eligibility for remote work is established by agency management and is subject to change at their discretion at any time and for any reason.
Some travel throughout the state of Utah may be required.
