Health Program Specialist III
The Division of Services for People with Disabilities is seeking a dedicated professional for the Health Program Specialist III role. This position provides ongoing support coordination and case management for individuals receiving DSPD services. The Health Program Specialist III works directly with people and their teams to assess needs, develop person centered plans, and ensure that services are delivered appropriately and consistently. The role includes coordinating providers, monitoring service effectiveness, addressing barriers, and helping individuals maintain stability in their home and community. This position focuses on service oversight, problem solving, crisis coordination, and ensuring that supports remain aligned with each person’s goals, preferences, and safety needs.
Resolves a variety of complex issues and may perform a combination of the following tasks. Develops, interprets, and implements policies. Ensures enforcement and compliance with federal and state rules, regulations, and mandates. Provides quality assurance and monitors program effectiveness. Provides education, communicate and educate contracted community providers and staff, care recipients, and the pubic. Work closely with other state agencies on Medicaid eligibility.
