Healthcare Navigation Hub Personal Health Navigator (CSP-PHN)
About the Job Summary:
The Clinical Support Provider Personal Health Navigator (CSP-PHN) at WVP Health Authority is Traditional Healthcare Worker who serves as a liaison between client, providers, other medical staff and community resource organizations. The CSP-PHN is a position that works interdependently with the rest of the CSP team, other health care professionals, and community partners to support equitable and quality care to the client. The CSP-PHN will document interactions with clients in EHR system for tracking and coordination. Additionally, coordinates with CSP-Clinical Coordinator for intake and processing of referral information, including screening, insurance verification, and appointment scheduling for clients. CSP- PHN will create their own clinical schedule based on the need of the referral volume, will engage in community events to promote the Health Navigation Hub services and gather services for clients served.
JOB REQUIREMENTS:
TYPICAL PHYSICAL DEMANDS: Work required sitting for long periods of time, in office environment as well as driving to other locations in the community to meet clients. Occasionally lifting of supplies weighting up to 20 pounds. Is required to speak and hear sufficiently to communicate either via phone, virtual or in-person. Must be bilingual in Spanish.
HOURS: This is an hourly position. Work hours are generally 8:00 AM – 5:00 PM with 1 hour allotted lunch, Monday through Friday. Occasional work hours that may extend before and after normal business hours may be needed to fulfill business needs.
Primary Responsibilities:
- Providing direct resource management to their clients.
- Provide updated case load lists to their supervisor weekly.
- Review new incoming referrals and communicate with referral source for updates and coordination.
- Perform documentation of client interactions within EHR system timely.
- Build and maintaining relationships with treatment other community resources organizations
- Maintain documentation, status reports, statistical data and other required documents
- Presentations in the community about Healthcare Navigation Hub
- Perform other duties as assigned.

Qualifications:
- Must have THW certification and be able to apply for Personal Health Navigator through OHA.
- Ability to pass a background check for being on public school premises.
- Current Oregon Driver’s License and a good driving record.
- Bilingual in English and Spanish.
- Ability to effectively interact with co-workers, members who have a diverse ethnic or cultural backgrounds, religious views, political affiliations, lifestyles, and sexual orientation and treat each with respect and dignity.
