Job Description:
- The Postpartum Care Connector supports a health equity–focused program that guides mothers through a 12-week postpartum care period. This role engages patients in the hospital setting and provides ongoing follow-up and support primarily through phone outreach. The Care Connector builds trusted relationships, connects patients to community and health system resources, and supports overall postpartum wellbeing. Candidates with experience in community-based, maternal health roles are strongly preferred, as this position is ideal for a compassionate, mission-driven individual committed to improving maternal health outcomes.
- Promotes excellent customer services through interactions with patients, physicians, care managers, and the healthcare team.
- Collaborates with the Care Managers for patient care coordination needs. Coordinates patient care and services arrangements under the direction of a RN or SW Care Manager and in collaboration with the patient/family.
- Provides and documents the delivery of regulatory letters to patients or their representatives in accordance with Federal and State regulations.
- Arranges patient transportation and follow up appointments as requested.
- Obtains insurance authorization for patient post-acute care needs as directed by the Care Manager. Other duties as assigned.
Knowledge, Skills, and Abilities:
- Intermediate to advanced computer skills required to include, but not limited to Microsoft Office, Word, Outlook, Power Point and Excel.
- Excellent interpersonal and communication skills are required.
- Medical terminology helpful.
- Good public relations/clerical and computer skills.
Education:
- High School Grad or Equiv [Required]
Field of Study:
Work Experience:
- 1+ office experience [Preferred]
- 1+ office experience, preferably in a medical setting [Preferred]
- Previous data entry experience [Preferred]
