Serenity Care PACE (Program of All-Inclusive Care for the Elderly) is seeking a compassionate and dedicated Master of Social Work (MSW) to join our interdisciplinary team in Springfield, MA. This is a full-time salary-based position offering meaningful work that supports aging adults in maintaining independence and dignity within their communities.
Responsibilities:
- Conduct psychosocial assessments and develop individualized care plans.
- Provide counseling, advocacy, and support services for participants and families.
- Collaborate with an interdisciplinary team to coordinate holistic care.
- Assist with crisis intervention, discharge planning, and resource referrals.
- Ensure compliance with federal and state regulations and documentation standards.
Qualifications:
- Master’s Degree in Social Work (MSW) required.
- Active Massachusetts LCSW or LICSW license preferred.
- Experience with geriatric, long-term care, or PACE populations highly desirable.
- Strong interpersonal, organizational, and communication skills.
- Bilingual a plus, but not required.
