Serenity Care PACE (Program of All-Inclusive Care for the Elderly) is seeking a compassionate and dedicated Master of Social Work (MSW) to join our interdisciplinary team in Springfield, MA. This is a full-time salary-based position offering meaningful work that supports aging adults in maintaining independence and dignity within their communities.

Responsibilities:

Conduct psychosocial assessments and develop individualized care plans.
Provide counseling, advocacy, and support services for participants and families.
Collaborate with an interdisciplinary team to coordinate holistic care.
Assist with crisis intervention, discharge planning, and resource referrals.
Ensure compliance with federal and state regulations and documentation standards.

Qualifications:

Master’s Degree in Social Work (MSW) required.
Active Massachusetts LCSW or LICSW license preferred.
Experience with geriatric, long-term care, or PACE populations highly desirable.
Strong interpersonal, organizational, and communication skills.
Bilingual a plus, but not required.

Pay: $80,000.00 - $90,000.00 per year
