Mountain Community Health Partnership (MCHP) is seeking a full time Behavioral Health Provider to join our team. The Provider will deliver behavioral health services in collaboration with primary care providers, focusing on underserved, uninsured, and high-risk populations. Key Responsibilities:
- Provide integrated behavioral health services during or in coordination with primary care visits.
- Utilize motivational interviewing techniques to assess patient readiness for change and provide brief interventions.
- Conduct behavioral health screenings, including managing the SBIRT (Screening, Brief Intervention, and Referral to Treatment) program for depression and substance use.
- Coordinate and facilitate group medical visits for patients with chronic conditions.
- Review patient records to assess behavioral health history and identify opportunities for health behavior change.
- Participate in interdisciplinary care team meetings and daily "huddles" to coordinate patient care.
- Maintain compliance with HIPAA, infection control, and safety protocols.
Qualifications:
- Education: Master of Social Work (MSW) or equivalent. Valid and current license to practice in NC.
- Experience: Minimum 2 years of supervised clinical experience, preferably in a primary care setting.
- Strong clinical assessment, communication, and teamwork skills. Proficiency in Microsoft Office. Flexibility to travel between clinic locations and work occasional evenings.
