The Role
At Shape Health, being a coach isn’t about handing out quick fixes—it’s about guiding people back to the foundations of true health. You’ll help members reclaim energy, focus, and vitality by addressing what really matters: how they eat, how they move, how they rest, and how they manage stress.
This role is about connection and impact. You’ll meet members where they are and walk alongside them as they build new habits and discover their own power to heal and thrive.
Be a catalyst for change—help each person rewrite their health story one step at a time. Backed by a team of medical providers, therapists, and community programs, you’ll be at the center of our mission: creating real transformation through trust, accountability, and human connection.
Responsibilities
- Build strong, trusting relationships with members through one-on-one coaching, group sessions, and group-based activities.
- Guide members in setting and achieving goals around nutrition, fitness activity, sleep, stress management, and overall well-being.
- Provide nutrition and behavior-change coaching grounded in proven approaches, helping members make dietary shifts step by step and understand how food choices affect health outcomes.
- Provide education and accountability while adapting plans to fit each member’s culture, lifestyle, and personal challenges.
- Meet with clients virtually, by phone, in their home, or out in the community.
- Connect members with appropriate providers, programs, and community supports; coordinate referrals and follow-up care.
- Support members in building health literacy and self-advocacy skills through education and coaching.
- Track progress on measurable health outcomes (weight, nutrition habits, blood pressure, mental well-being, etc.) and adjust plans as needed.
- Provide moral support and model healthy behaviors to encourage sustainable lifestyle change.
- Lead and support group coaching, group workout clubs, group yoga sessions, or cooking classes to strengthen community connection.
- Maintain accurate and timely documentation of coaching interactions in our digital platform.
- Collaborate with medical providers, therapists, and community health workers to ensure members receive whole-person care.
- Help members navigate health and social service systems, including SNAP, WIC, and other community resources.
Must-Haves
- Passion for helping people improve their health and well-being
- Strong interpersonal and communication skills with the ability to build trust
- Experience in nutrition, fitness, health education, or related fields
- Ability to motivate and hold members accountable with empathy and encouragement
- Comfortable using technology for scheduling, documentation, and virtual coaching
- Reliable transportation to meet members in homes or community spaces
Nice-to-Haves
- Relevant certifications such as NBC-HWC, CNC, RDN, CWC, IHC, ICF, CHC, CPT, or CHW
- Professional experience working in healthcare
- Experience coaching in the asynchronous setting
- Experience using behavior change methods
- Bilingual (Spanish/English)
- Knowledge of motivational interviewing or behavior-change techniques
- Familiarity with chronic condition management (e.g., diabetes, hypertension, postpartum health)
- Group facilitation experience (cooking demos, yoga, fitness classes, support groups, etc.)

Job Types: Full-time, Part-time, Contract
Pay: $30.00 - $50.00 per hour
Expected hours: 10 – 30 per week
