Position Summary

Note: All employees of Cascadia Healthcare are required to submit and be cleared to work in the facility per each state’s specific background check requirements prior to contact with patients/residents.

Essential Functions

Assessing and admitting eligible patients Education staff, physicians, referrals sources of Hospice criteria Meeting productivity of admissions per day
Documentation accuracy per Medicare guidelines.
Experience hospice or related field desired
Respond to inquiries from facility discharge planners, families and other referral sources.
Punctuality and regular attendance for assigned shifts. This position is day shift but will occasionally require more than 40 hours and evening work.

Other Functions

Ensures compliance with company’s clinical policies and procedures.
Follows company standards, policies and procedures and sound judgment.
Performs other related duties, as assigned.

Knowledge/Skills/Abilities

Must maintain confidentiality and protect resident rights, regarding all resident/personnel information.
Strong knowledge of HIPAA laws and related regulations.
Must possess strong communication, customer service, teamwork and organizational skills.
Positive , enthusiastic attitude.

Qualifications

Education

Prefer a Bachelors

Licenses/Certification

No license required

Valid CPR Certification.

Experience

Six months experience in a long-term care environment preferred.

Job Type: Full-time

Pay: $25.00 - $33.00 per hour

Expected hours: 40 per week
