Job Summary
The CM/CBRS Provider plays a vital role in supporting individuals with Mental Health Diagnosis and their families. This position involves providing direct support, fostering independence, and enhancing the quality of life for clients through personal Treatment Plans and interventions. The ideal candidate will have a Degree in a Human Service Field of some kind, is a Team player and will be committed to empowering individuals to achieve their personal goals.

Requirements:

Must be able to pass a background check
Have reliable transportation and a valid drivers license
Be a Licensed Social Worker or at least have a 4 year degree in a Human Service field

Responsibilities:

Develop personalized treatment plans
Work one on one with participants to reach their goal
Work with participants to maintain or gain their independence in the community
Maintain client information and abide by all HIPAA regulations.

Job Types: Full-time, Part-time

Pay: $19.98 - $24.06 per hour

Expected hours: 20 – 40 per week
