Responsibilities
- Function autonomously to care for patients with acute, critical and chronic health conditions.
- Coordinate care management proactively working with the healthcare team and facility partners to produce good outcomes.
- Provide medical oversight of patient care and rehabilitation.
- Ensure proper progress on plan of care and identify any early warning signs for readmissions during the critical, immediate post-discharge period
- Coordinate appointment for evaluation by specialist as needed for patients during their stay at the care facility
- Work individually and in interdisciplinary teams of resources and consultants
- Facilitates patient transition within and between health care organizations to ensure accurate communication among the health care providers
- Record and maintain patient records in timely manner to ensure all services are accurately accounted for and available for review by the interdisciplinary teams
- Timely documentation, discharge summaries, medicine reconciliation, etc. in EMR
- Talk to patients about effectively managing their health, as well as assisting in designing appropriate treatment plans and recommending further care

- High level of clinical proficiency and excellent decision-making skills.
- Ability to work independently and as an effective member of a healthcare team.
- Excellent oral and written communication skills.
- Ability to effectively collaborate and build relationships with patients, families and all care providers
- Proven organizational skills and ability to prioritize effectively.
- Strong computer skills with knowledge and experience with Electronic Medical Records
Qualifications
- Current unrestricted licence MD/DO
- Current and unrestricted DEA certificate
- Board certified or board eligible
- BCLS/ACLS desired
