Position Locations: We have openings for multiple positions across Putnam and Dutchess Counties.
Shift: 9 a.m. – 5 p.m., plus one evening per week is required. This role is hybrid and involves field work.
Job Responsibilities:
In this role, you will coordinate and participate in activities related to care management services for family members and caregivers. Northwell Health’s Health Home Serving Children is a New York State Medicaid care coordination program that provides access to medical, mental health, substance abuse, social support services, and family support services to children who need them. Health Home services are free to those who qualify.

Job Qualifications:
- One to three (1-3) years of relevant experience, required.
- Bachelor’s degree or master’s in social work or a relevant field strongly preferred.
- Two (2) years’ experience in home health or care management strongly preferred.
- Driver’s license valid in New York State, strongly preferred.
- High school diploma or equivalent required.
The salary range for this position is $44,450 - $69,340 per year*.
