Key Responsibilities

Listen to clients and understand their needs, concerns, and goals.
Help connect individuals and families to helpful resources, programs, and services in their community.
Provide encouragement, emotional support, and practical guidance.
Conduct virtual check-ins, intake conversations, and follow-ups.
Keep accurate and up-to-date notes on client interactions.
Work alongside healthcare providers, coordinators, and other team members to ensure clients feel fully supported.
Stay aware of available community resources and programs to better serve clients.

What We’re Looking For

A genuine people-person who loves building rapport and making others feel comfortable.
Strong empathy and the ability to connect with clients from all walks of life.
Excellent communication and listening skills.
Comfortable working virtually using video calls, phone, and online tools.
Organized and reliable — able to follow through on commitments and keep clear records.
Prior experience in customer service, caregiving, coaching, or a related role is a plus, but not required.
