Patient Care Coordinator – Part-time (100% Remote, Potential for Full-Time)

Momentous Health provides online cognitive evaluations to geriatric patients across 42 states. We are seeking a Patient Care Coordinator to join our team. This is a remote role with a minimum of 20 hours per week and potential to expand into full-time for the right fit. In this role, you will serve as the first point of contact for patients and their families, guiding them through scheduling, intake, and follow-up with empathy and professionalism. You will also support our clinicians by managing referrals and ensuring accurate, timely communication with providers.

Key Responsibilities:

Patient Support & Scheduling

Schedule and confirm patient appointments.
Assist patients with technology setup for telehealth sessions.
Respond to patient inquiries via phone and email with compassion and professionalism.
Manage patient intake paperwork

Referral & Provider Coordination

Manage incoming referrals from our partners.
Communicate with physicians and clinical staff.
Maintain accurate documentation of referrals.

Administrative Support

Verify patient insurance coverage and submit claims accurately.
Maintain and update patient records with accuracy and confidentiality.
Coordinate communication between patients, families, and clinical staff.

Required Qualifications:

Minimum of 3 years of experience in patient care coordination, medical office administration, or healthcare support strongly preferred.
Excellent communication and interpersonal skills, with the ability to support patients and providers.
Strong organizational skills and ability to manage multiple tasks efficiently.
Comfort with technology, including telehealth platforms, and willingness to guide patients through setup.
Ability to work independently in a fully remote environment.
