Care Coordinator/Care Navigator - Neurodiversity Support

OverviewWe are seeking a compassionate and detail-oriented Care Coordinator/Care Navigator to support caregivers of children with neurodiversity/neurodevelopmental disorders, particularly autism, as well as individuals on the autism spectrum. This role requires a unique blend of compassion, organizational skills, and knowledge of neurodevelopmental disorders. The ideal candidate will be dedicated to improving the lives of individuals with neurodiversity and their caregivers, providing both emotional support and practical assistance throughout their care journey.

Key Responsibilities

Client Support and Advocacy

Provide empathetic listening and moral support to caregivers and individuals with neurodevelopmental disorders.
Conduct initial assessments to understand client needs, challenges, and goals.
Develop personalized care plans and checklists tailored to each client’s unique situation.
Offer guidance on available resources across educational, medical, government, and social services sectors.
Serve as a resource for education and support for families.

Care Plan Development and Coordination

Provide care plan development support to parents of children with disabilities, including education, coordination, and consultation, as well as administrative duties.
Facilitate shared decision-making and collaborative teams for the child’s care in terms of education, health, and other services.
Collaborate with parents in the assessment and ongoing reassessment of the child’s needs, maintaining daily and monthly updates.

Resource Navigation and Coordination

Assist clients in accessing and understanding self-screening tools.
Guide clients through the diagnosis process, including finding appropriate specialists.
Help identify and connect clients with suitable healthcare providers, therapists, and support services.
Facilitate appointment scheduling and follow-ups.
Assist in completing necessary forms and paperwork.

Care Team Collaboration

Coordinate communication between various care team members (e.g., healthcare providers, educators, therapists).
Attend and participate in care team meetings as needed.
Ensure consistent information flow among all stakeholders.

Education and Information Sharing

Educate clients about neurodiversity, available therapies, and support options.
Keep clients informed about relevant community resources, events, and educational opportunities.

Document Management

Help clients organize and maintain medical documents and records in a secure, collaborative manner.
Assist in creating and updating comprehensive care binders.

Task Management and Follow-up

Track and follow up on assigned tasks and activities for clients.
Provide reminders and support to ensure adherence to care plans.
Monitor progress and adjust plans as needed.

One-on-One Family Support

Partner with parents to map out where they may increase the resources available to them, including insurance, public benefits, and the school system.
Provide one-on-one support via messaging and video chat when families have questions, decisions, or wins to share.
Utilize our database of expert-driven articles, videos, and step-by-step guides to assist families with topics that matter most.
Support families with tools such as our web and mobile apps, alongside specialists in public benefits, insurance, and special education.

Emotional Support

Provide a compassionate, listening ear to clients facing challenges.
Offer coping strategies and connect clients with appropriate neuro/mental health resources when needed.

Qualifications

Bachelor’s degree in Psychology, Social Work, Education, or a related field; Master’s degree preferred.
3+ years of experience in care coordination, case management, or related roles, preferably in neurodevelopmental disorders or autism spectrum.
Strong knowledge of autism spectrum disorders, neurodiversity, and available support services.
Excellent interpersonal and communication skills, with a compassionate and patient demeanor.
Ability to work effectively with diverse populations and in multidisciplinary teams.
Strong organizational and time management skills.
Proficiency in using electronic health records and care management software.
Familiarity with HIPAA regulations and commitment to maintaining client confidentiality.

Preferred Skills

Experience in behavioral therapy or applied behavior analysis (ABA).
Knowledge of educational rights and IEP processes.
Familiarity with government assistance programs and eligibility requirements.
Bilingual skills (e.g., Spanish, Mandarin, ASL) a plus.

Job Types: Part-time, Contract, Per diem

Pay: $24.22 - $50.00 per hour

Expected hours: 4 – 20 per week
