Help Me Grow SC is dedicated to promoting the healthy development of children from prenatal through age 5. We are seeking a passionate and experienced Care Coordinator to join our team. The ideal candidate will have a strong background in early intervention and child development, will play a crucial role in assisting parents and caregivers seeking support for their child's developmental and behavioral needs.
Position Overview:
HMG is hiring a Care Coordinator positions within our Centralized Access Point (CAP) team, serving as the primary point of contact for families, caregivers, and early childhood professionals seeking guidance on child development and behavior. The Care Coordinator will assess needs, coordinate referrals to community-based programs and services, and maintain efficient operations through database management and virtual collaboration.
Key Responsibilities:
- Family and Professional Support: Respond to inquiries, provide guidance on early childhood development, and support families navigating complex systems.
- Referral and Coordination: Process referrals, conduct intakes and screenings, and connect families to relevant community resources.
- Client Interaction and Documentation: Maintain accurate documentation of client interactions and discuss screening results with families.
- Professional Development: Participate in training, workshops, and quality improvement projects.
- Operational Efficiency: Manage multiple databases and optimize workflows in a virtual office setting.
Job Requirements:
- In-depth understanding of typical and atypical early child development.
- Relevant experience serving young children and families.
- Strong communication, technological proficiency, and cultural sensitivity.
- Commitment to professionalism, confidentiality, and equity.
Preferred Experience and Qualifications in Early Care and Education:
- Demonstrated knowledge of South Carolina’s early care and education systems, resources, and stakeholders.
- Experience working in or providing support to child development centers or family childcare programs.
- Experience collaborating with ECE training and technical assistance providers.
- Familiarity with the DSS Division of Early Care and Education and its programs.
- Strong organizational, interpersonal, and communication skills.
- Proficiency with database management and virtual collaboration tools.
